The nutrition support program provides home parenteral and enteral (tube feeding) support to individuals who cannot consume or absorb nutrients through regular food intake. As a Silver Circle Partner, we help strengthen the foundation’s educational and outreach efforts to home nutrition patients.
With Coram, you will have the benefit of a home nutrition support team dedicated to enhancing your care. Your local Coram dietitian, pharmacist and nurse will work together with you and your family to make sure you are fully educated and comfortable with receiving nutrition therapy in your home. They will monitor your individual nutrition progress and work with you to improve your quality of life.
Coram's nutrition support program includes intradialytic parenteral nutrition (IDPN). This parenteral nutrition solution is for dialysis patients who are malnourished, or at risk of malnutrition, and who are unable to meet their nutritional needs by oral diet, tube feeding, or daily parenteral nutrition.
We partner with both patients and members of their care teams to meet each patient’s nutritional needs. Coram's team of nutrition support specialists (registered dietitians, pharmacists and nurses) can provide clinical support in identifying appropriate candidates for IDPN, ordering IDPN, and monitoring the response to therapy.Coram’s staff will review your needs to ensure that you have the education, clinical resources, and the products and services needed to enhance your quality of life.
At the start of care, our specialized tube feeding team will provide you with a comprehensive nutrition screening, identifying any potential nutritional risks that require further attention. The team will establish a baseline for monitoring your response to therapy. Our registered dietitians will complete a detailed nutrition assessment and share their recommendations with your physician. As therapy progresses, your team will monitor you to assess compliance and response to therapy, and to identify forms of feeding intolerance (to help avoid potential dehydration or weight loss).
