Coram Education In order to assure positive outcomes, Coram’s objective is to teach patients and/or their care partners how to safely administer infusion therapy in their home environment and become an integral part of their own care delivery.

Coram is committed to Continuity of Care. Our patient education program begins as soon as possible, preferably prior to or during hospitalization or the initiation of therapy.  It is our experience that this increases the patient’s familiarity with home infusion therapy and the equipment and procedures to be used. The result is that patient and family apprehensions are greatly reduced, learning is enhanced and compliance improved – all leading to a greater probability of the desired clinical outcome.

Coram’s program includes:

  • Patient/care partner education materials, which include pictures and written, step-by-step instructions
  • Personalized training by nurse clinicians (patients and/or care partners verify knowledge through return demonstration.)
  • Nursing visits to reassess techniques and reinforcement training as needed
  • 24-hour access to Coram clinicians
  • Home Patient Service Representatives  (HPR) to act as the patient’s liaison for inventory, supply and delivery management
